All our Support workers have years of experience with current National Police checks, Working With Children Checks, First Aid Certificates, Covid 19 Vaccinations and training for your peace of mind.
How it works
- You can contact us directly OR we receive a referral from your coordinator
- We will speak over the phone with you and discuss how we can accommodate the supports based on your goals. You get to ask us questions to feel confident about the services.
- Next step is to know if the total budget in your NDIS plan is there to provide you with the NDIS supports you need for your disability and help you work towards your goals. You can help us with this part, or your coordinator will inform us in your referral.
- Once you are satisfied, we can arrange a time to send someone out for a quick assessment and our service agreement OR if you want an immediate start then you can help one of our team members fill out some questionnaires over the phone to create a comprehensive care plan. We would then send a Service agreement via DocuSign on your email. Once we receive the signed service agreement back via DocuSign, a Support Worker or a Registered Nurse will be rostered to provide support services to you.
- We will then contact you within 24hrs and provide you with the details of the Support worker. We will allocate a Support workers that aligns with your support needs.

Transitioning From Hospital to Home
It can be very difficult and challenging to access quality homecare to maintain your independence at home. Our compassionate and understanding staff are highly experienced in supporting you at home once you have been discharged from the hospital into the community.
We work together in partnership with you and create individualised care plans to meet each individual’s needs.
Once discharged from hospital our staff can help you step by step to understand and assist with the process of settling back at home.
Our qualified and experienced staff have the ability to identify and recognise individual care to suit all types of clients. Their expertise will help your transition from hospital to home much smoother by adapting your discharge plans and catering to match your needs of care at home.
Whether it is pre or post operative surgical care, to requiring assistance at home with household tasks, to taking you to your appointments, our staff have knowledge and experience in supporting you in all areas.
Compassionate nursing and disability services staff will cater to your individual care needs at home and will encourage and support you to maintain your independence at home.
